* I understand that my dog will be groomed which includes shampoo, conditioner, ears cleaned, nails trimmed and anal glands expressed. While hypo allergenic products are used allergic reactions can occur. I will be contacted immediately in the event of a reaction.

* I understand that equipment may have to be used to restrain my dog including harnesses, tie downs or muzzles if my dog gives the signal that it intends to bite.

* I understand payment is due at drop off but additional charges may be incurred due to coat condition. Additional charges are typically for aggressive dogs or dogs that are extremely matted or soiled. These dogs will have their pictures taken before and after the groom so there will be no question as to condition.

* I understand that my animal may have a pre-existing condition (such as a heart condition or epilepsy) that could be exacerbated by being groomed. I will be called immediately in the event a problem arises.

* I understand that dogs in a community environment can get colds and other infectious ailments.

* I understand that Mile High Mutts, Inc. will take every precaution by using proper restraint to prevent nicks and cuts caused by scissors and clippers but accidents can happen when dealing with wiggly animals. I will either be notified immediately if they are serious in nature or I will be told at pick up.

* I understand my animal will never be tranquilized unless it is provided to us and was prescribed by a vet.

* I understand that if I leave my dog for a period of three days without word to Mile High Mutts, Inc. they will consider my dog abandoned and appropriate measures will be taken.

* I agree to be solely responsible for any and all acts or behavior of my pet while in the care of Mile High Mutts, Inc. If my pet should become ill, injured or seem to be in need of medical consideration Mile High Mutts, Inc. reserves the right to administer aid and/or to use any available veterinarian. I, the owner, shall pay any expenses so incurred.
